Tutta Bella Scholar-Athletes
Scholar-Athlete Nomination Form (COACHES ONLY) The Tutta Bella Scholar-Athlete Awards are given out at the end of each season (Fall, Winter, Spring) to one outstanding athlete in each sport who embodies leadership and success both athletically and academically. Nominations come from the coaches. See Tutta Bella Athlete Awards, under Student Recognition for more information.
Booster Parent Reps / Coaches / Advisors
*IMPORTANT: Financial requests (payments, reimbursements & transfers) will only be fulfilled if made from the email of the Booster Parent Rep of that specific program. Requests made by Coaches or other parents will not be paid.
Request form: Booster reimbursement, Invoice payment, ASB transfer: Reimbursement requests can be made from the Booster general fund or a club/activity/sport's designated account. If requested funds are from a club/activity sport designated account, ONLY requests submitted by the Booster Parent Rep will be processed & reviewed. By completing this form, funds can be transferred from a designated Booster account to a designated ASB account but cannot be transferred back.
Fundraising Request Form: This form is for clubs and sports wishing to add a fundraising line item to their budget. Please note: Booster Club can only approve fundraisers that students are not directly involved in (Pass the Hat, parent-only events, etc.). All other fundraisers (car wash, food or item sales, etc.) must run through ASB.
Booster Club Facilities Request Process: Any IHS program under ASB that has reserved funds in their Booster Account, may use the Booster Club to reserve ISD Facilities. The costs of the reservation are the responsibility of the program.
